Jason Shirk ff5609785b Minimize writing ANSI escape sequences in PSReadline (#4110) 
Instead of changing Console.*groundColor on every character, only set
the color when the color changes - this avoids writing out ANSI escape
sequences after every character.

Fixes #4075

Jason Shirk cc664bc639 Fix lock contention during script compilation (#3064) 
In rare cases, it was possible to cause high contention on a lock used
while compiling code to run in the interpreter.

This fixes the problem in a couple different ways:

* Use ConditionalWeakTable which has finer granularity in locking
* Avoid dictionary lookups in the most common cases
  - There really aren't too many real cases, I could have covered them
    all, but keeping the code generic is useful for the future.

Not quite related, but I noticed we didn't cache certain interpreter
instructions and it made sense to add a cache because we generate a lot
of array-init instructions.

Jason Shirk 7cd5935595 Use correct win32 console mode w/ external commands (#2991) 
At startup, we need to save the current console mode before potentially
enabling VT100 support.

Then, when running an external command, we should use that initial
console mode instead of whatever mode we changed it to.

An application can't rely on the shell enabling VT100 because enabling
by default can cause problems for some applications, therefore
Windows applications must enable VT100 themselves.

Jason Shirk bd9e49d8d9 Make Out-Null fast (#2629) 
People use Out-Null despite much faster alternatives:

    $null = Do-Stuff
    [void] = Do-Stuff
    Do-Stuff > $null

This change makes Out-Null work in roughly the same say as the above.

The optimization is to detect that we're calling the built-in Out-Null
cmdlet when invoking a pipeline from script (the change won't have any
effect in the PowerShell api). If we detect Out-Null, we rewrite the
pipe to look similar to `Do-Stuff > $null`.

Jason Shirk c8c55b7ce7 Fix NullReferenceException in binder (#2591) 
There was a NullReferenceException after turning on constrained language mode
if creating a binding on a thread with no runspace.

The fix is to assume if there is no runspace that we are not in constrained language
mode, and also generate a binding restriction that assumes the same.

Jason Shirk cf18010ad5 Fix multi-line input w/ redirected stdin (#2090) 
The input loop reading from stdin did not handle multi-line input correctly
because it was adding a newline character where none was expected.

The fix was to not include the final newline character to accept input,
just like Console.ReadLine or PSReadline would.
