Sergei Vorobev f3ab8b3f57 Enhance mappings story 
- Split mappings.json into mappings.json for every project (folder under
  src)
- Throw away src\monad prefix, now it's not relevant
- Remove New-MappingsFile
- Rename Copy-SubmoduleFiles to Copy-MappedFiles
- Make Copy-MappedFiles flexible about set of mappings.json that it
  uses

Sergei Vorobev bf1a2c8521 Make Add-Type work without a flat folder structure 
- Get rid of the assumption that all framework assemlbies live in
  the same place
- Enable build scenario (assemlbies are referenced directly from unpacked
  nuget packages)
- Fix #766
- Re-enable Add-Type tests

There are 2 functions:

- New-MappingFile - creates mapping.json from project.json files
  (one is included in this commit)

- Copy-SubmoduleFiles - enables copying files back and forth from
  submodule to src/<projects> based on mapping.json file

Add almost all files to Microsoft.PowerShell.Commands.Management
One of them is Computer.cs that was listed in known issues.
We start to use a nuget packages generated for assemlbies that
cannot be listed in framework assemlbies, but exist in a GAC
on all windows machine and not a PowerShell assemblies
The first one is Microsoft.WSMan.Management

Building sln file generated by cmake causes troubles on my dev machine.
It contains additional checks for tools version that cannot be satisfied.

Sergei Vorobev f8abd74658 Fix for #491: make powershell.exe work in new window 
FullCLR windows host doesn't read keys when run in separate window.
Fix: don't use Start-Process -Wait $false -NoNewWindow $false

Instead, ommit this parameters. Seems like a bug in Start-Process.

This change brings a very minimal set of tests and make
build fail, if we cannot load some PS assamblies on FullCLR

We should increase test coverage for FullCLR in future.

Sergei Vorobev a3c149196c Make build.FullCLR.ps1 build RELEASE by default 
Since we start include CRT runtime inside powershell.exe,
the size of executable jumped from 32kb to 1Mb in DEBUG.
Release produces about 20kb, so we keep size managable.

AppVeyor is a cloud-based CI platform for windows.
Use [skip ci] tag in any place of message to mark commit as trivial (i.e.
README.md changes).
