Major changes are:
- Make all commands return 'ConfirmImpact.None' if `SupportsShouldProcess` is not set to `true`.
- Update some cmdlets to explicitly use `ConfirmImpact.Low`.
- Update `DefaultCommands.Tests.ps1` to test for 'ConfirmImpact' level.

Testing

The tests are organized by testing language. Thus Pester tests, which are written in the PowerShell language, are in ./powershell and xUnit tests, written in C#, are in ./xUnit. The third-party shebang test is in ./shebang.
